Lip Flip or Lip Filler: Which is Best for You?

If you want a little more definition or fullness in your lips but still want to look like yourself, you may be wondering whether to choose a lip flip or lip filler.

A lip flip uses Botox to relax part of the muscle around the mouth, allowing more of the upper lip to show. Lip filler adds volume and can refine shape and definition. Both can create subtle enhancement, but they work differently, and the best choice depends on your anatomy, goals, and daily activities.

At The Fresh Method in Bel Air, Maryland, we begin with a conversation. Whether this is your first injectable treatment or you have had your lips treated before, you deserve time to ask questions and understand your options.

What is a Botox lip flip?

A lip flip uses small Botox injections near the upper lip to relax part of the orbicularis oris (the muscle that surrounds the mouth). This allows the lip to turn outward slightly, revealing more of the pink lip without adding filler. The change develops gradually rather than appearing immediately. A lip flip is an off-label, but very common, use of Botox.

For someone nervous about filler, a lip flip can be a shorter-term introduction to lip enhancement. However, it does not preview exactly what filler would look or feel like.

Why we ask what you do for a living before a lip flip

Before recommending a lip flip, NP Ellen will ask how you use your lips throughout the day.

Do you speak publicly? Play the saxophone or another wind instrument? Does your work depend on precise lip control?

Because a lip flip changes muscle activity, tasks such as sipping, pursing the lips, or controlling a straw may temporarily feel different. If that would interfere with your work, we may choose a different approach or discuss timing around a break from those activities.

What does lip filler do?

Hyaluronic acid lip filler adds volume and can improve shape, definition, or balance. It can be used for a modest change; choosing filler does NOT necessarily mean choosing dramatically larger lips.

At your consultation, we may discuss:

  • Restoring fullness lost over time.

  • Defining the lip border.

  • Improving selected asymmetries.

  • Creating a softer, fuller appearance.

  • Addressing certain fine lines close to the lip border, when appropriate.

Nurse Practitioner Ellen Lancaster will assess your lips both at rest and when you smile. She also looks at how they relate to the rest of your face. The goal is an enhancement that looks beautiful with lipstick and without it, and a result feels proportionate from your perspective as well. After all, beauty is subjective, so the most important goal to accomplish is your goal! If gum show or irritation around mouth creases is your main concern, we will assess the cause separately rather than assuming lip filler is the solution.

Which treatment looks more natural?

Either treatment can look very subtle and natural. A lip flip is not automatically the more natural choice, and filler is not automatically a dramatic one.

The important questions are what needs to change, how much change you want, and which treatment can reasonably accomplish it.

If you want more upper lip to show without adding volume, we may discuss a lip flip. If you want additional fullness, definition, or restored volume, filler may be a better fit.

We choose the amount based on your anatomy and goals. There is no need to chase a larger result simply because more product is available.

Does lip filler hurt?

The initial needle pokes can be uncomfortable. At The Fresh Method, we offer topical numbing cream before treatment, and the lip fillers we use contain lidocaine to help reduce discomfort as treatment progresses. Individual experiences vary.

Many injectors have had their own lips treated, so they understand why comfort matters. NP Ellen uses numbing cream, gentle technique, distraction, and pauses when needed. Patients often thank us for that extra attention.

A lip flip involves only a couple of quick injections. While it is a very quick procedure, numbing cream is available for that treatment, too!

What should you expect after treatment?

After lip filler

Swelling begins early, so the lips you see immediately after treatment are not your final result. Bruising and tenderness are also possible.

In our practice, we prepare patients for swelling that may be more noticeable the next day before gradually improving. We invite patients back around three weeks after treatment to review their results and compare photographs.

*Pro tip: Give routine swelling time to settle before deciding how you feel about the appearance. However, always resch out to your injector with any concerning symptoms.*

Filler does carry uncommon but serious risks, including blocked blood vessels, tissue injury, and vision loss. Seek immediate medical attention for unusual or worsening pain, pale or dusky skin, or vision changes. Contact your injector promptly about any unexpected symptoms. A tthe fresh method, our policy is "there is no such thing as a dumb question, and there is no such thing as bothering us!" We want our patients to know that we are always happy to answer questions, even if they are repeat questions.

We also provide preparation and aftercare instructions. Plan for bruising, follow your specific instructions, and avoid strenuous exercise for at least the first 24 hours unless advised otherwise. Do not stop prescribed medications to reduce bruising without approval from your prescribing clinician.

After a lip flip

Recovery is usually less visibly noticeable than with filler, though injection-site bruising or swelling can occur. Temporary changes in lip control can develop as Botox takes effect. Unlike hyaluronic acid filler, Botox cannot be dissolved; its effect must wear off. Lip flips only last a couple of weeks, as opposed to filler that lasts months. Therefore, we often recommend lip flips to start for those who are very nervous or skeptical about the results.

How long do results last?

Lip flips are shorter-lived than lip filler, but neither treatment has one guaranteed timeline.

At The Fresh Method, we counsel patients that the lip-flip effect may feel quite brief, usually only weeks. Published estimates may be longer, so we discuss the dose, your goals, and the possibility that your experience may differ.

For first-time filler patients, we use approximately six months as a practical point to reassess the visible result. That is not an expiration date or an automatic recommendation to add more. Patients most often find their enhancement remains noticeable longer with subsequent treatments. The maintenance plan depends on what is still present, how the lips look, and whether more treatment would be beneficial. Product and treatment area do affect longevity.

*Pro tip: You do not need to commit to the same amount of filler every six months or every year. We reassess rather than refill by the calendar.*
